Résumé :

La mise en place, à partir de 2002, d’un accès gratuit et quasi universel aux thérapies antirétrovirales pour les personnes vivant avec le VIH/sida au Botswana offre un observatoire privilégié des transformations des politiques internationales de santé au Sud. Le partenariat entre le gouvernement du Botswana, la compagnie pharmaceutique Merck & Co. et la Fondation Bill et Melinda Gates qui s’est noué grâce à une collaboration de recherche biomédicale entre l’école de santé publique de Harvard et le gouvernement botswanais révèle l’imbrication des enjeux philanthropiques, scientifiques et de santé publique. En étant déterminée par son accessibilité, sa faisabilité et ses chances de réussite, la santé globale tend à reproduire différentes matrices d’inégalités au sein des systèmes de santé et au niveau global.

Abstract:

By focusing on access to free antiretroviral treatments in Botswana, this article analyses recent transformations of international cooperation on health issues. The partnership between the government of Botswana, the pharmaceutical company Merck & Co and the Bill and Melinda Gates Foundation was made possible through pre-existing biomedical collaboration between the government of Botswana and the Harvard School of Public Health. This configuration of actors and interests offers a vantage point of contemporary global health partnerships in which philanthropy, science and public health overlap. Most often determined by their convenience, accessibility and chance of success, global health interventions tend to reproduce inequalities within health systems and on a global level.
